Nationality & Domicile

  • Government-issued ID (e.g., PhilSys National ID) — must still be valid
  • Domicile in: Metro Manila Region, Cavite, Rizal, Batangas, Laguna, Bulacan, or Pampanga

Employment Status

  • Salaried employee — private company / GOCC / min. 2-year contract
  • Minimum fixed income ₱25,000/month
  • Last education certificate
  • Other documents as outlined in the IPA

  • Latest 3 months’ salary slips

Documents to Prepare

Stage 1 (Application)

Government-issued ID

Any valid ID such as PhilSys ID, Passport, UMID, SSS, GSIS, Driver’s License, or Voter’s ID.

Statement of Account (SOA)

Latest billing or SOA from your current creditors.

Bank statements (latest 6 months)

Your latest 6 months, submitted in PDF format.

Annual tax document (ITR)

Your latest ITR filing (Form 2316 or the equivalent from your employer).

Salary slips (latest 3 months)

Your latest 3 months of payslips.

Stage 2 (Verification)

Proof of address

Lease contract, electricity or water bill, or barangay certificate of residency.

Certificate of Employment & TIN

A valid COE from your employer plus your active TIN.

Last education certificate / diploma

Certificate or diploma of your highest educational attainment.

Existing loan agreements

Signed agreements or contracts for any loans you currently have.

Auto-debit authorization form

The signed DDA form so repayments can be auto deducted.
